I’d like to spend a little time discussing why I combine fat transfer with facial aging surgery. And it’s really a very obvious process that we’ve only known about and become aware about fairly recently. And that’s the concept that as we grow older, yes gravity wins, and yes, things fall. But that’s not the only thing that happens; our faces get thinner as we get older. They get a lot thinner as we get older, so if I’m on a diet and I lose weight, my clothes are going to get big. And as we age and as we lose some of the natural baby fat from our face that’s part of the reason that the face falls and the skin becomes loose. The giant mistake that many surgeons make is that they try to fix everything by pulling; by being a tailor and taking in the clothes and tightening the skin of the face. Well that’s wrong. What you create is a tight skeleton. If you think of children they don’t have tight skin in their faces, but they have soft round contours and baby fat and thicker skin. Almost always, with very few exceptions, when I do an aging face procedure, such as a facelift or a neck lift, I will always talk to the patient about fat transfer, because I’d like to gain a little weight before I take the clothes in, I’d like to gain the shape of the face back before I do any tightening. Those two things combines is what creates that beautiful, natural, and un-operated look. I’m Dr. Lawrence Koplin, and thank you for listening.
